Home and Community Care Program for Younger People (HACC PYP) Services

What we do

The Home and Community Care Program for Younger People (HACC PYP) provides support services to people under 65 (under 50 for Aboriginal and Torres Strait Islander people) with disabilities, chronic illnesses, or short-term health needs, helping them live independently at home.

Bayside Health offers the following services for people in living in Bass Coast Shire:

Community Nursing

Community Nursing covers District Nursing, Palliative Care, Diabetes Education, Continence Management, Cardio/Pulmonary care, Stomal Therapy and Wound Care. These are delivered by specialist District Nurses, Clinical Nurse Consultants and Nurse Practitioners who can visit patients in their homes or in community settings.

Allied Health Services

Allied Health services include Physiotherapy, Podiatry, Dietetics, Occupational Therapy, Social Work, Speech Pathology and Counselling. These services help people maintain function, manage conditions and stay as independent as possible.

Meals on Wheels

A home delivery meal service for people who are unable to prepare their own food due to health or mobility challenges.

Social Support Groups

These groups provide social connection and activities for people who may be at risk of isolation, supporting mental and emotional wellbeing alongside physical health.

Transport

We also provide transport to and from medical appointments coordinated through our volunteer transport program.

People under 65 years of age (or under 50 for Aboriginal and Torres Strait Islander people) with disabilities, chronic illnesses or short-term health needs.
