Volunteer transport (medical)

Volunteer transport assistance for eligible Bass Coast residents attending essential medical appointments

The Bayside Health Volunteer Program provides a transport service for eligible consumers of Bass Coast Shire who need to attend medical appointments, visit specialists, be admitted to hospital or be taken to a chemist.

This service is for people who are unable to drive or are unable to use public transport.

What we offer

Volunteer transport is booked on a first-in basis once the consumer meets eligibility criteria and fulfils a risk assessment.

The program only operates Monday to Friday.

The booking office is open weekdays, 9am-4pm.

How to access this service

If you are over 65 and have a Support at Home Package, you will need to contact your Care Partner and ask them to send us a Service Request form.

If you are aged over 65 and don’t have a Support at Home Package, you will need a transport referral code through My Aged Care (phone 1800 200 422).

If you are aged under 65, you will require a letter of support/request from your General Practitioner or Social Worker.

To help us accommodate as many people as possible, if you no longer require the service, please phone the office on (03) 5671 3372. Please note that 48 hours' notice is required where possible or cancellation fees may apply.

How much will it cost

Fees will be discussed with you at the time of requesting transport.

We are supported by the Commonwealth Home Support Program (CHSP) and Home and Community Care (HACC) to provide subsidised transport and to support clients transitioning to the Support at Home Program (formerly Home Care Packages (HCP)).

From 1 November 2025, the new Aged Care Act 2024 is in effect.

This new law strengthens your rights, improves quality and safety, and ensures that all aged care providers, including Bayside Health, are open and accountable.
