District Nursing brings compassionate and community-based health support to your home wherever you are in the Bass Coast Shire

What we do

Bayside Health’s District Nursing Service provides a range of clinical nursing needs for people in their own home, supporting independence and improving health and wellbeing.

District Nursing Service covers all of the Bass Coast Shire and recognises that every individual has different needs and is considerate of cultural diversity.

Our dedicated and passionate team provides care that is tailored to client needs and is delivered by a team of trained, competent and experienced Registered and Enrolled Nurses.

What we offer

The District Nursing Service can provide:

  • Wound care management
  • Continence care
  • Medication management
  • Client support in stoma care
  • Assistance with diabetes management
  • Drain tube management

There might be a cost to the service depending on the patient’s circumstances, eg Department of Veterans’ Affairs. 

How to access this service

Referral to this service can be made by your general practitioner (GP), another medical specialist or a healthcare professional, as well as hospital-based staff in the emergency department, inpatient wards and other areas of the hospital.

We will contact you within clinically recommended timeframes. If there’s no waiting list, you’ll receive an appointment booking letter or we’ll call to arrange a convenient time.

Clinically recommended guidance and resources for Health Professionals in the Gippsland region are available through Gippsland Pathways.

Additional information is required relevant to the referral, eg. recent scans and test results, specialist letters or wound care plan.

More information

Patients in public hospitals are eligible for post-acute care on discharge. This includes free visits by a District Nurse for 28 days (there is a cost for consumables required, consumables would typically be bandages, dressings, IV fluids, tubing, masks, gloves, etc).

The District Nursing intake office is available Monday to Friday, excluding public holidays, 7:00am to 3:30pm.
