Cardiopulmonary rehabilitation

The Clinical Nurse Consultant offers education and individual consultations for people who have asthma or any other respiratory disease. 

What we do

Our Cardiopulmonary Rehabilitation Program provides comprehensive, supervised care by our nursing and allied health teams for individuals living with heart and lung conditions. We offer Heart Failure Rehabilitation and Pulmonary Rehabilitation, designed to improve quality of life, increase physical capacity, and support long-term health.

Our Cardiopulmonary Rehabilitation Program helps individuals with chronic lung or heart conditions to manage their condition and stay active.  The programs include an eight-week exercise and education program coordinated by the Clinical Nurse and conducted by a multi-disciplinary team to help clients to reach their goals.

Healthy heart (heart failure) rehabilitation

Our Healthy Heart program supports patients with chronic cardiac conditions through structured exercise, education, and monitoring

Who is it for

  • Individuals diagnosed with heart failure
  • Patients recovering from cardiac events or procedures
  • Those experiencing reduced exercise tolerance or fatigue

What we offer

  • Supervised exercise training tailored to your condition
  • Heart health education (medications, diet, symptom management)
  • Monitoring of vital signs and symptoms
  • Lifestyle counselling and risk factor modification

Pulmonary rehabilitation program

How to access this service

For clients with respiratory disease or cardiac conditions, a doctor or other health professional's referral is preferred, although self-referrals are accepted.
