The Support at Home program has been providing participants with choice in services, support, and care needs in their local area, across Cardinia Shire, Gippsland, and a small area of Casey Shire since 2017.
A dedicated team of Care partners and support staff work with you and or your representative to develop a unique care plan designed to suit your needs, with innovative solutions and services.

There are many benefits to connecting with us, as we are a non-profit public health organisation with health professionals connected to services and support outside of the support at home funding to assist you in staying independent in your home.
