Transition Care helps you regain your functional independence and confidence, potentially avoiding the need for longer-term care and support

What we do

The Transition Care Program (TCP) supports your recovery following hospitalisation through short-term specialist care designed to restore independence and confidence whilst reducing reliance on long-term services.

We understand returning home after hospital can feel overwhelming for older Australians. Our TCP provides essential support during this critical transition period, ensuring you're adequately prepared for home life or, when appropriate, residential aged care placement.

Your individualised care plan reflects your specific needs and personal goals. Our multidisciplinary team includes physiotherapists, dietitians, occupational therapists and social workers, working alongside our experienced nursing staff to deliver comprehensive, compassionate care.

The program focuses on rebuilding practical skills, managing medications, addressing mobility concerns and establishing community connections.

We work collaboratively with you and your family to achieve sustainable outcomes that support your wellbeing and independence.

How to access this service

All prospective residents must complete an Expression of Interest Form and an aged care assessment.
