We may need to provide ongoing care to monitor your condition and support your recovery, ensuring you feel safe and well looked after during this important transition post operation.

Our Kooweerup Hospital team is here to work alongside you, helping you regain your strength and independence at a pace that's right for you. Before you are discharged from hospital, the staff will have a conversation with you about what happens next and whether you will need post-acute care.

They will take the time to explain what kind of support might be helpful for you and answer any questions or concerns you may have. We want you to feel informed and prepared as you move forward, knowing that you have the care and resources you need to recover comfortably in the familiarity of your own home.
