What we do
Research demonstrates that patients recover faster and more successfully in their home environment, experiencing fewer complications such as infection, delirium, or confusion compared to hospital-based recovery.
However, the program is not suitable for all patients, as some individuals require higher levels of care during their recovery period. The service accepts DVA Health Cards (both Gold and White) as payment when accompanied by a GP referral, making it accessible to Department of Veterans' Affairs clients. Patients remain under their hospital doctor's care throughout the program while receiving hands-on nursing support in their homes.
If you qualify for in-the-home services after your hospital visit, we will assign district nurses to support you on the road to recovery. You will remain under the care of your hospital doctor during this time, but our nurses will always be on hand to help.
District nurses are assigned to support patients throughout their recovery journey, providing caring and qualified assistance.
