Hospital Admission Risk Program (HARP)

HARP supports people with chronic conditions through coordinated care to reduce avoidable hospital visits

What they do

Hospital Admission Risk Program (HARP) provides specialised person-centered care coordination for people with chronic disease and/or complex needs, including self-management, support strategies and networks.

HARP is a free program that aims to prevent avoidable hospital admissions and emergency department presentations.

What is provided?

HARP provides short-term care coordination. Your clinical care coordinator will provide information, support and education to you and your family/carer regarding your health condition and help coordinate services such as My Aged Care. Working with you and liaising with other health professional services, an individual management plan will be developed to best meet your care needs.

HARP also coordinates funding for home oxygen referrals post-hospital discharge.

Eligibility

The program cares for people who live in Bass Coast and South Gippsland, and are at risk of avoidable hospital admissions and emergency department presentations.
