What we do
Cancer can be a daunting experience that has long lasting impacts well beyond treatment.
Bayside Health Regional Care Group Cancer Survivorship Program recognises the importance of creating a supportive network for you to thrive post treatment.
We will focus on providing information on how to reduce the short- and long-term side effects of your treatment, and will spend time understanding the broader physical, emotional, social and practical impacts that you may require support with.
We will ensure that all health professionals involved in the delivery of your care are kept informed and will try to provide you with supports as close to home as possible.
How does the program work?
You will have an appointment with a Survivorship Nurse who will guide you through a series of questions to better understand your needs.
Together you will develop a Survivorship Care Plan to identify your after-treatment goals and the supports most important to you.
The Survivorship Carer will provide you with a range of resources and information, including referrals and contact details of service providers that can help meet your needs.
Your Survivorship Nurse will agree on a follow-up schedule with you to ensure that you have timely access to the services and supports you will need.
Referral from your GP
To access this service, you’ll need a referral letter from your GP or medical practitioner.

We will contact you within clinically recommended timeframes. If there’s no waiting list, you’ll receive an appointment booking letter or we’ll call to arrange a convenient time.
Access to the Program is via a referral. This can be from yourself, a family member or:
- Your GP, Specialist or Oncologist
- Allied Health professional
- Social worker
- Pharmacist
- Other Survivorship programs
What are the benefits?
The focus of Survivorship Program is to help you thrive post treatment. With the right referrals and information you will be able to:
- Monitor possible long-term side effects
- Receive timely access to support
- Adjust well to life after treatment
The Cancer Survivorship Program is provided free of charge. Referrals made to other service providers may incur a cost.
Any community member of the South Gippsland Shire who has completed cancer treatment is eligible.
Where is the service provided?
Depending on your needs the service can be provided to you:
In-clinic at Korumburra, Leongatha or Tarwin Lower;
or at home; or via telehealth.
