Transition care program

A short-term, goal-oriented and person-centred service restoring independence to your life after a hospital stay

What we do

The Transition Care Program (TCP) is a joint Commonwealth and State/Territory Government funded program that aims to provide short term support and active management for older people who have been in hospital.

Transition Care is for older persons who have been in hospital or rehabilitation and assessed by the Aged Care Assessment Service (ACAS) as being eligible for the Transition Care Program.

TCP aims to improve your independence and confidence, providing you with the opportunity to return to normal activities after a hospital stay.

A range of services are offered as part of an ongoing care plan. TCP can be accessed for a maximum of 12 weeks, giving you and your family time to consider and plan for your long-term care needs. The average length of stay in the TCP is 5-6 weeks.

TCP offers case management, low intensity therapy and personal support allowing people to have greater opportunity and support to achieve recuperative goals, optimise independence and function, and finalise longer term care arrangements.How to access this service .

What to bring with you

Every time you come:

  • Medicare card
  • Health Care Card and/or concession card (if you have one)
  • Private health insurance card (if applicable/if you want to use it)
  • Adverse drug alert card (if you have one
  • Previous& X-ray films, scans, ultrasounds or any other test results or reports
  • Medicines you need to take while you are here
  • List of medicines you are currently taking (or the boxes), including medicines you have bought without a prescription, such as herbal supplements and vitamins
  • Glasses, hearing aid(s) and/or walking frame if needed

Bringing these items helps us give you the best care possible and ensures your visit goes smoothly.

We also recommend bringing these:

  • Comfortable day clothes and footwear (preferably not slip on shoes) and night / sleep wear
  • Personal hygiene items, such as shampoo, shaving equipment, toothpaste and deodorant
  • Something to do, like a book to read, a magazine or an iPad or tablet with headphones

What not to bring

  • Large amounts of cash
  • Valuables, such as expensive jewellery

Will I need to pay?

TCP is supported by federal and state government funding. To help deliver your care, GSHS asks you to contribute a daily fee.

This amount is set as a percentage of your income, equivalent to:

  • 17.5% of the single age pension per day if you receive care while living at home or in the community (approx. $13 per day);
  • 85% of the single age pension per day if you receive care while living in a residential/multifunction setting (approx. $62 per day).

Where is it?

Although Transition Care may be provided in a person’s home in other catchments, at this time, Bayside Health Regional Care Group offers solely a residential, bed-based Transition Care program at:
