Our aim is to provide high quality care to all patients and their families through the palliative phase of their illness

What we do

We offer comprehensive inpatient palliative care services designed to provide high quality, compassionate care to patients and their families during the palliative phase of illness. Our dedicated approach ensures dignity, comfort, and support throughout this important journey.

Our experienced Palliative Care Specialist Nurse works collaboratively with you and your loved ones to ensure that all comfort needs are met, with particular attention to effective pain management and symptom control. We understand that palliative care extends beyond physical needs, which is why we provide comprehensive psychological and emotional support for both patients and their families during this challenging time.

Recognising the demanding nature of caring for a loved one, we offer palliative care respite services, providing carers with essential breaks to rest and recharge while ensuring their loved one receives excellent care.

We maintain a strong partnership with Palliative Care Southeast to address complex clinical situations, including specialised pain management requirements. Our team of district nurses works in close co-ordination with these specialists to facilitate seamless transitions from hospital to home, ensuring continuity of care and support every step of the way.

Referral from your GP

To access this service, you’ll need a referral letter from your GP or medical practitioner.

We will contact you within clinically recommended timeframes. If there’s no waiting list, you’ll receive an appointment booking letter or we’ll call to arrange a convenient time.

What to bring

Every time you come:

  • Medicare card
  • Health Care Card and/or concession card (if you have one)
  • Private health insurance card (if applicable/if you want to use it)
  • Adverse drug alert card (if you have one
  • Previous& X-ray films, scans, ultrasounds or any other test results or reports
  • Medicines you need to take while you are here
  • List of medicines you are currently taking (or the boxes), including medicines you have bought without a prescription, such as herbal supplements and vitamins
  • Glasses, hearing aid(s) and/or walking frame if needed

For a clinic appointment:

  • Your appointment letter
  • Any special items listed on your letter
  • TAC or WorkCover claim number (if relevant)

Bringing these items helps us give you the best care possible and ensures your visit goes smoothly.
